We have a take home form that we use. It looks like this.We also ask for business info for future events.

Next year we would like to really involve the parents in the classrooms and school. The parent group has discussed having several big events, fundraisers, class parties, etc. We all feel it’s a great way to build our classroom resources and school spirit!
One way is by asking parents to fill out this form so we have information about you, your student(s), and your business.
Also, Next year we need Room Parents who would be willing to assist each teacher. We would like to have a volunteer parent(s)/guardian(s) (Grandparents welcome!) to help with parties, scholastics, and any other fun event(s) we have. These parents would be in charge of sending letters home to help teachers collect donations such as cookies, paper goods, drinks and volunteers for these classroom events.
We truly feel we can have a great year and include these fun days,
But we need your help!!
