I have built a simple database in MS Access to store and report parent skill info. I also have a cover letter and survey form to go with it. If you are familiar with Access (it's part of the MS Office suite), and would like a copy of this material, send me an email. Our survey won't go out until next week, so this hasn't been actually used yet, but I didn't want to wait another month to offer it since there's so many requests for similar tools.
