This looks to be an old topic - but I wanted to let parents know that we send home a parent volunteer sheet we refer to as our VIP (Very Important Parents) Volunteer Sign Up Form. I would love to share, it has worked out great for us and we receive a ton of them back - probably 200 at least each year. We then enter them into a database and print lists for each activity as they come up. That list then goes to the volunteer coordinator for calling and scheuduling. It has worked very well. We took the results of this form and entered it into a database application to publish a PTyellOw pages for our elementary school.

It was a fair bit of work the first year, but should be pretty quick for next.

If you highlight the parent survey you can
copy and paste into a blank word document.
I don't know if anyone knew that you could do that. Then save the document under whatever file
you want. Then go back in and hightlight again and change the document to a normal instead of
normal web. That will help default your fonts. Hope this helps.
