We do an end of year survey that lists all of our activities with a 1-5 scale. We state a simple thought at the top like "rate the activites below according to your experience with them at MBES and then put simple statements like

I thought the Carnival was a great event:

1-Strongly disagree, 2 disagree 3-neither agree nor disagree 4 agree 5 strongly agree

and then we add some write in space at the end that allow the parents to list their top PTO programs and also a wish list for future stuff. This year we are going to encorporate a kids portion as well so they can let us know what they think...d

You know, the guy that was chairman of our Fall Carnival wants to do the same thing. He can't figure out if this event is just for fun or if it's a fundraiser.

One thing I would put on your questionnaire if you do send one out:
"Would you be willing to help with next year's event?" People are ready and willing to critique something, but when you ask them if they want to help, they hesitate.

IF your event was a success, I wouldn't fool with it. People will always have opinions of how something could be done better.

We have just finished our annual winter carnival (big success this year) and we are thinking about sending out a questionnaire to the parents (and possibly staff) to see what likes and dislikes they may have.

Has anyone ever done this before? We would like to get feedback, but on the other hand, we are not sure what we may be getting ourselves into.

Any comments or suggestions would be greatly appreciated.
