We made up[ our own on word. We talked with the t-shirt company about the size and color. For us it was a plain white t-shirt with the school name on the front left chest and on the back it was a big 06' and inside of that all the kids signed their name. Our order form had a clip art t-shirt on the top and then the lines for the kids name, teacher,and sizes for them to choose. Because we had shirts that ran small our sizes were Youth L, Adult SM, MED, LG, & XL. Just keep it fun as long as you know how many you need it doesn't need to be formal! Have Fun!

i am in brain freeze. Does anyone have a copy of their t-shirt order form they can email me? The (local) company that we order shirts from does not have their own forms and the last PTO destroyed all of their old forms. i can't for the life of me get it right. Between different colors, sizes and prices, i am just melting down.
I just need a picture to stimulate my creativeness.
