volunteer interest survey

I'm not sure if this is exactly what you want, but this is what our PTO is putting on our membership letter(we soooo did not have this together last year):

At the top, we give a short description of who we are (PTO), what our mission is, and what we'd like to accomplish for the year - any special projects, etc.

We then have an area that lists the different activities that we have planned for the year with two check boxes next to them with a little message that says "please check any area(s) you'd be willing to help" The first box is to show which activity they're interested in and the second box is labeled "committee chair".

It would look something like this:

Interested committee chair
Membership drive ______ ______
Family Fun Nights ______ ______
Fall Festival ______ ______

You get the idea. We will also of course ask for each person's address, phone number, email address, and how they would prefer to be contacted. We're planning to make contact with every person within the first week (even if they only signed up to help with teacher appreciation) just so they know that we want to stay in touch. I've talked to the officers about this (after reading something similar to this online here) and we discussed how we had about 70 people enrolled as members, but only about 20 that were consistently at meetings (which I have learned here is not as important as I thought)or that helped with other activities. I think a lot of people signed up at open house, but just said 'call me if you need me' and we never really did or at least not until the end of the year. We're hoping to improve involvement by making the first contact by phone, say "Thank you...I see you'd like to help with...someone will contact you around this time.....do you think you'd like to help out with anything else OR do you think you'd like to chair/co-chair that committee when it comes around? Are there any activities you'd like to see added (if so, would you be willing to chair?)

Sorry for rambling-we're still working out some of the details. Hope this was what you needed.
