Hi. I've been asked to develop a form to send home to parents over the summer to try to get more parental involvement in our parent club (our version of a PTO). Last year we had to cancel many events due to lack of volunteers. Our parent club is down to about 4-5 members and we're afraid this year isn't going to be any easier. We simply can't do it by ourselves. It's been a very hectic summer for me and I really need to get this project done soon. I am just sitting here with my mind a blank slate. I've been trying to make this volunteer survey for days and I'm just lost.
I would love to see examples of other PTO's volunteer sign up forms, surveys etc. Could someone spark my imagination by sharing their examples? What I'm wanting to do is send something home telling the things we've done, the events we sponsor, and ask for help with certain events throughout the year.
