Here's what I made up when I was President this past year. I used the free survey service at
www.zoomerang.com, and emailed all PTO members who had given me their addresses. For those members who did not provide an email address, I printed out copies, and sent them home via the "backpack express".

PTO PRESIDENT'S SURVEY 2004
1. What grades are your children in at Hill Country? Head Start Pre-K K 1 2 3 4 5 None here
(circle all that apply)

2. Our PTO does fundraisers to pay for different projects, teacher requests, and activities. What would you like to see the earned money used for? Circle all the ideas you like.

A. Playground improvements
B. Paved jogging track
C. Family activities (Game Night,Reading Night,
Career Night, etc...)
D. Cafeteria noise reduction
E. Poster-making machine (banners, signs,
publicity ads, etc...)
F. Library supplies/materials
G. Teaching supplies/materials
H. Additional field trips
I. Educational assemblies

J. Walkway cover between the new wing and 5th
grade hallway
K. Walkway cover for front drop-off area
L. Computers, software, technology-related
items
M. Other idea (please tell us!)

3. Of the choices your circled, which two would you give the highest priority?

4. PTOs use all sorts of ways to raise money for their schools. Circle the letters of the ones that you would be willing to support here at Hill Country Elementary:

Out of 172 surveys sent, I got 88 replies--not too bad. I tallied the results, and then let everyone know the top 3 vote-getters in what to do with the money, and the top 3 fundraiser ideas. We then worked hard to accomplish as many of the members' top choices as possible. (I hope this info lines up clearly; sorry if it does not). ;)

We sent a survey out last year asking parents what they wanted to do for fundraising, but I never thought of asking them what they would like to see the PTO purchase. GREAT idea!
