There are a few parent and teacher survey's post on this site. www.orgsites.com/ma/ptoinfo
they are in MS word so you can cut and paste what you want

I just posted a parent involvement survey that can be printed and used for your purposes of gathering information about volunteers. You might want to add some specific questions about your PAL program.
Also, I think you could survey teachers and ask things, such as, Do you discuss homework with parents (these will all be yes, no answers), Do you talk to parents about the learning style of their child, do parents know the class schedule or school day of the child, Do parents understand the academic standards and report card, Do parents feel welcome in your class, Do parents know how to contact you, Do parnets know the importance of being involved in their child's learning, Do you welcome parents suggestions, Do you find something for ech parnt to do to contribute to the classroom, etc. Things along those lines will get teachers really thinking about how they involve parents with the child's school life.
I would list and number each question and have a box for YES and another for NO to the right. Add in columns and lines and there ya go, you've got a survey!

I'd do have them write the answer for more distinct feedback. If they can support extra information on the survey for improvement, it better helps your school. Try:

Do you have any suggestions for improvement? If so, please take the time to list them below.
___________________________________________________________________________________________________________________________________________________

What do you like about our school? Would you like us to emphasize these programs?
___________________________________________________________________________________________________________________________________________________

What do you dislike? Could we change/remove these and make improvements? Are these your opinions, or are these your child's?
___________________________________________________________________________________________________________________________________________________

Would you like to add any other information to complete our survey? We appreciate feedback.
___________________________________________________________________________________________________________________________________________________

(If you run out of room, you may write complete answers on the opposite side.)

I am the volunteer coordinator for an elementary school and I would like to send out a survey for the teachers and another one for the volunteers to see what they liked and disliked about the volunteer PAL / Room Parent program. I would like any questions that I could ask the teachers and some for the volunteers. Also, what type of format should I use. ex: 1-5 format(with 1 being dislike the most) or ask a question and have them write the answer?? HELP!!
