I know just what you need and my copy (someone from here send me) crashed with my computer last year BUT mainly it asked for parent info (address, e-mail,etc) and at the bottom a simple check off for talents and interests. Nothing fancy.

The form was kept simple and easy to read. We even had a few grandparents sign up (mainly for if we need food and BOY did they come through [img]smile.gif[/img] ).

I'm the volunteer coordinator for an elementary school opening up this fall. At my old school, the volunteer sign-up sheet was geared towards specific events--things like check here if you were interested in working the book fair, bingo, science night, etc.

There is no way that we are going to have a comprehensive plan of what we are going to do at the new school until September at the earliest, although many things will be worked out over the summer.

So--I'm trying to come up with a volunteer sign-up sheet that we can give to parents now that is much more general in nature. Things like check here if you can volunteer at school during the day on a regular basis, here if you are proficient in certain computer programs, here if you are willing to work with any classroom, here if you want to work on a fundraiser, etc. And then ask if you have a particular interest in any certain program or event that you have seen in your old school. This way, as things shape up over the summer we know the best people to call.

Anyway, I'm plugging away at this, but I'm wondering if anyone has something similar to this that I can borrow from? I would appreciate it!
