here is a parent survey i wrote for our school. until we sent these home we were winging it!! we have 5 'official members' of the pto but got 40 of these returned by parents eager to help out but don't want to sit through meetings that last 1-2 hours and addresses their area of interest for only 5min! who can blame them? i created a table with 4 sections and pasted this info in each. that way if you have a 4th grader and a 7th grader in the same school the pto knows specifically what areas you'd like to get involved in for each grade and all of the appropriate people have your number. make it cute and make it yours! good luck!
