Our membership form is quite basic: Parents Name
Phone Number, address, students name and teacher. We attach it to a very "inspiring" welcome letter at the start of each school year. We also had membership cards made up from a company that only charges for shipping and handling. They are just business card size that we fill in the famliy name and school year.
