click on PTO link and then forms. We send the membership form AND the emergency notification form out on the 2nd day of school with a 2 week turnaround.

See below. You can cut and paste, change the formatting a bit if you need to. We make this double sided and add the volunteer form on the back. This way parents only need to submit one form.


Membership and Directory form:
ONLY ONE FORM PER FAMILY NEEDS TO BE SUBMITTED.
Please enter all children/class/grade so that they can be included in the friend finder. Send this completed form, along with a $10 check per family made payable to '___' PTO, to the classroom teacher in an envelope marked:
"___ PTO Membership" by September 30.

Hi -

Is anyone willing to share examples of their membership forms to join the PTO. Our new board is playing major cleanup after some..let's say "lazy" years. Any help would be so appreciated!!:D
