We use relaybuilder.com for our website and it has a survey creator. To be honest, we have not used it yet.

I would loke to do a low cost or no cost survey with the option for busy parents to complete it online. The last paper survey had a 5% return rate.

I know that we will need to market it more but I also think an online survey is much easier for some parents.

Any experience?
