When I ran our Booster Club, we had a sponsor program. Basically, I came up with a template on Word that worked great. The bottom had a fill-out section for the Sponsors business name, address, zip, and phone number and an option of how to pay. Sometimes, Microsoft Office templates work as well. you can find those at www.microsoft.com

I'm looking for a sponsorship form for donations to bring to business. ie: $200=gold sponsor, $100=silver,etc, with a desription and maybe a tear off area.

Anyone got something close, I can manipulate?
