Look in the Parent/Family involvement section. They were talking about it there. You may even want to do a search for more. I've seen several on this site.
I would like to put together a parent questionnaire to help our PTO understand what their interests are so we can generate more attendance at our PTO meetings. We have many resources in our district and would like to utilize them for parent education purposes. I'm wondering where I could find information about developing a questionnaire or if their is a questionnaire out there I can use as a resource.
