At our harvest fling silent auction our form just simply asked for their name, phone number and bid amount. At our Breakfast with santa silent auction we used bid #s--people came to me were issued a number--i had a list with the bid number, their name, their phone number. On the actual auction forms they just put their number and bid amount---very simple either way.
