And, in NJ, you also need to have registered your PTO as a non-profit corp (if you plan to incorporate) with the state before banks allow you to open a bank account. This may hold true for some other states as well??

You can't open a new bank account for the new group without its own EIN. That's the SS-4 form - step one.

It was the 501c3 tax exempt but I guess I need the SS-4 one too. I am guessing we would need the SS-4 before transfering bank accounts to the new PTO name?

Can you be more specific? There's several IRS forms you might be looking for. Form SS-4 is for getting a tax id number for your group. Form 1023 is for applying to become a 501c3 tax exempt group. Form 990 or 990-EZ is the annual "tax" return your needs to file if it's a 501c3.

I thought I saw someplace on here the IRS form/book that needs to be filled out. I'm having trouble finding it now and can't remember the number of the form. Help?!
