Do you mean something along the lines of an order form? In past years, when we used Jostens, they provided us with order forms. This year, we've kept it very simple. The text is as follows:

This will be my first year collecting payments for the yearbook. Does anyone have a copy of their letter they send out to the student body, soliciting money for their yearbook that they can forward to me? I would appreciate it! TIA Kate
