Do you want sign up form or a database to catergorize the different areas that parents want to sign up for. There are two totally different forms. Let us know we need a little bit more explaination.

We have a volunteer form that we use at the beginning of the year that lists everything we will need parent volunteers along with a check box next to it and they are grouped by the time of the year (Fall, Spring, Etc.) We also leave room for parents to list special skills and anything they might want to bring to the school (science fair, career day). I can send it to you. However, I am in the process of making it a bit better for this year by adding the list of committees and getting parents to sign up right then for the ones they would like to be involved with...I will try and send the revised edition you way when it is done!!! ;)

Are you talking about gathering a database of volunteers? You can give them a list of all your committees and ask them to chose which ones they were interested in, or even have 1st, 2nd, 3rd choices. And/or ask what their talents are--good at organizing, greeting, baking, creating flyers, getting donations, etc. Then you can fit them into the right committee. Or ask if they want to be put on a committee or if they want to be called for specific events when needed. Tho if you did that, perhaps no one would actually volunteer for a committee! We are trying to update our volunteer forms. I would like to see some samples that others are using. I do not want to leave anything out but I also don't want to be too wordy. Thanks
