I tried something simmilar.
Not to the extent of fingerprinting, about two years ago. It was a form.
I merged the typical questions from the Youth Soccer, JA & employment applications into one. It had all the questions, other names, convicted of trafficing, Sex crime, DWI,.... With the disclaimer that our intent was to disqualify persons. Lawyers said no problem, Local and State police gave it the thumbs up. They pledged to run the names. It was all public information.
When I passed it to the District, the Volunteer cordinator thought it was "Over the Top" and "would discourage parent involvement".

Fingerprints might be over the top, at our own expense is a real slap in the face.

I do not see a questioneer as invasive, it is the norm in other youth activities.

How common is this requirement? Our school district started this a few years ago quoting a state law that doesn't really require this. We have to submit fingerprints to the State police clearing house every year at our own expense. Is that pretty universal for those of you who have this as well? And am I crazy but does this discourage parent involvement in the classroom even more? I'm anxious to hear your responses!
