Form SS-4 for EIN

Let's see
1-You would put your name as it is now, since you are not incporated you can not legally use that yet.

2-When we got ours it was because of banking purposes, we couldn't open our account without it, so I would think you would put other.

3-Line 10 we put the actually date of our very first meeting (it was 5 months prior to getting our number). This is the date you use until you are incorporated then you use that day as the day your business started.

4-Line 14 we put educational support group/parent group

We are just beginning the process of applying to become a 501c3 organization. We have written our by-laws and are ready to apply for our EIN. I have a few questions regarding form SS-4:
1. We plan to incorporate at the appropriate stage in this process. In line 1 do we put our group name as it appears on our by-laws? We can't include Inc. because we aren't yet, right? Do we need to ammend this form when we Do incorporate?
2. What is the appropriate response for line 9? I'm assuming "other" but we need to specify what.
3. Line 10 date business started - do we choose today or go back 3 years?
4. Line 14 - again I assume the response is "other" and maybe non-profit Parent-Teacher organization?

I guess that's it for now - I appreciate any help anyone can give me. I'd like to call it in so we can get a quick response. We want to open a new bank account using this new number.
