Use whatever name your group goes by. "PTO" mmight be the most common designation for independent parent groups, but that's not any sort of requirement. Lots of groups have their own name or acronym, different from the letters "PTO".

Ohhh...are you wondering if you need to put that sort of designation at all?...In our application, we spelled out Parent-Teacher Organization as part of our official name.
