PTO member survey

Survey that we are using for anyone that shows up to the next meeting. We are pretty much starting the group from scratch because of district changes, no old members, lost by-laws and pretty much no old information. Since we can make this group whatever we want to without problems of old rules etc. We wanted to get an idea of what the parents and teachers want/need and expect.
